Hemodialysis treatments NaturaLyte Liquid and GranuFlo Acid Concentrates, made by Fresenius Medical Care, have been used to treat many patients. In fact, up until March 2012, GranuFlo was the most widely-prescribed dry acid product in today’s dialysis industry. Problems with the warnings and instructions of these products led to inappropriate use of the products, which may have caused hundreds of dialysis patients to suffer heart attack, cardiopulmonary arrest or sudden cardiac death.

The Problem

Patients who require dialysis usually suffer from a condition called acidosis, which is a buildup of acid in the blood. Acid concentrates such as GranuFlo are used along with bicarbonate concentrates to neutralize the built up acid in the blood in a process called hemodialysis.

All hemodialysis products contain acetate, an ingredient that the body converts to bicarbonate. The problem with the Fresenius hemodialysis products GranuFlo and NaturaLyte is that they contain higher levels of acetate than most other concentrates. In 2010, Fresenius discovered that physicians weren’t taking into account the fact that these products contain higher amounts of acetate than other concentrates. By not taking this into account, the physicians were giving patients too much bicarbonate, which can result in a dangerous medical condition called metabolic alkalosis.

Reports have suggested that dialysis patients who were borderline for elevated pre-dialysis bicarbonate levels may have faced a six to eight times greater risk of cardiopulmonary arrest and sudden cardiac arrest in the dialysis clinics. According to an internal review conducted by Fresenius dialysis centers, during 2010 at least 941 patients reportedly suffered cardiac arrest during dialysis treatment. Even after the company became aware of the serious problem associated with its hemodialysis treatments, it failed to notify the FDA or issue a warning to all clinics that were using its products.

When Fresenius discovered the problem, it issued an internal memo to physicians practicing within its own centers and included instructions on how to safely use the product. According to reports, the company failed to notify physicians at other dialysis centers who used the products of the potential risk. The company is now under investigation by the FDA to determine whether it violated federal regulations by withholding information. GranuFlo and NaturaLyte were recalled on March 2012.
